Check the validity of a certificate

This form certifies the validity of your employee or student’s medical needs. The medical certificate shown to you has been issued by us following our Partner Pharmacist’s in-depth telehealth consultation with the patient.

While we cannot discuss the details of our call due to pharmacist-patient confidentiality, we do provide a reference number at the bottom of the medical certificate that can be entered along with the patient’s surname to check for validity.
